***CLINIC CLOSED ON THURSDAY MARCH 19th 2026***

Dr. Awan is currently away and will be back in office on April 6th, 2026. If your concern cannot wait until Dr. Awan returns, please consider the following options:

– Medical emergencies- please call 911 or go to your nearest emergency department.

– Urgent minor ailments- your local pharmacy may be able to assist with certain minor ailments, such as urinary infections, rashes, and minor eye conditions. Alternatively, you may contact our after-hours clinic for acute, urgent issues. Please refer to the ‘Services’ tab on our website for a list of conditions appropriate for the after-hours clinic and the appropriate contact number. Please note that routine matters, paperwork, and ongoing issues will not be addressed by the after-hours clinic.

– Other concerns- you may seek care at a local walk-in clinic if you are unable to wait for Dr. Awan’s return.

Patient Registration

New Patients Registration:

New Patients should register through Health Care Connect Process.

Patients currently registered with another physician:

Call Service Ontario Deroster line at 1-866-532-3161, have a health card ready to complete deroster.
Wait for a formal letter in the mail about 7-10 days to confirm you have successfully derostered from previous FD
Call the Health Care Connect registration line at 1-800-445-1822 to become an active patient in the HealthCare Connection registry. Advise them that you want to be registered with Dr. Malik Awan at White Rose Medical Centre.
Call RN Health Care Connector Katherine at 1-800-263-3877 X 5449, provide her Health Card Number so that patient can be found in the database to confirm
Patients will then be contacted by White Rose Medical Centre to book an initial meet and greet with the doctor
