Patient Registration
New Patients Registration:
New Patients should register through Health Care Connect Process.
- Call Service Ontario Deroster line at 1-866-532-3161, have a health card ready to complete deroster.
- Wait for a formal letter in the mail about 7-10 days to confirm you have successfully derostered from previous FD
- Call the Health Care Connect registration line at 1-800-445-1822 to become an active patient in the HealthCare Connection registry. Advise them that you want to be registered with Dr. Malik Awan at White Rose Medical Centre.
- Call RN Health Care Connector Katherine at 1-800-263-3877 X 5449, provide her Health Card Number so that patient can be found in the database to confirm
- Patients will then be contacted by White Rose Medical Centre to book an initial meet and greet with the doctor

We also offer uninsured services which are NOT covered under OHIP.
