New Patients should register through Health Care Connect Process.
Call Service Ontario Deroster line at 1-866-532-3161, have a health card ready to complete deroster.
Wait for formal letter in the mail about 7-10 days to confirm you have successfully derostered from previous FD
Call the Health Care Connect registration line at 1-800-445-1822 to become an active patient in the HealthCare Connection registry. Advise them you want to be registered with Dr. Malik Awan at White Rose Medical Centre.
Call RN Health Care Connector Katherine at 1-800-263-3877 X 5449, provide Health Card Number so that patient can be found in the database to confirm
Patients will then be contacted by White Rose Medical Centre to book an initial meet and greet with the doctor

After Hours Clinic
White Rose Medical Centre is part of the Cobourg Family Health Group. We offer a virtual after hours clinic, Monday to Thursday from 5-8 pm and Saturday OR Sunday from 9am to 12pm
The after hours clinic is for URGENT ISSUES ONLY that cannot wait until the next office day. Please see below for more guidance on when to use the after hours clinic.
Prescriptions for narcotics or other controlled substances will NOT be issued by the after hours clinic physician, as they must be obtained through your own regular Family Physician.
Note: The after-hours clinic service is ONLY for enrolled patients of Physicians that are a part of the Cobourg Family Health Organization.
